Job Description

The Abramson Center Transitional Care Unit (TCU) has an exciting, leadership opportunity for a Full-Time Director of Clinical Services currently available.

This position is responsible for the management of the TCU and includes  planning/coordinating clinical activities in order to provide the highest level of patient satisfaction and care.

The Director of Clinical Services will:

  • Provide leadership for clinical and administrative functions in the TCU
  • Review RAPS and triggers from MDS to generate individualized care plans
  • Establish plans to correct deficiencies found during quarterly compliance checks 
  • Review incidents and complete DOH reports as needed
  • Maintain TCU in a regulatory compliant state at all times
  • Evaluate patients to ensure appropriateness of admission in conjunction with the clinical team
  • Maximize community referrals
  • Form effective physician and community referral relationships
  • Review and manage TCU budget

The ideal candidate for this position will have firsthand knowledge of DOH quality measures, quality assurance, performance improvement plans, CMS regulations, and the new survey process.

Job Requirements

Experience, Education, and Licensure

Minimum Experience: 3+ years of experience in transitional care with prior management experience.

Minimum Education: Nursing Degree (Associates Degree or Diploma)

Preferred Education: Baccalaureate in Nursing

Licensure/Certification/Registration: Valid PA RN Licensure. IV certification preferred; CPR certification.
