Abramson Senior Care is seeking a RN, Private Duty Case Manager for clinical support at our Jenkintown, PA office. The RN Private Duty Case Manager is responsible for planning, coordinating and supervising professional paraprofessional staff (CNA’s, HHA’s, Friendly Visitors, and/or home support staff, assigned to an Abramson Home Care client in a designated geographical area.

The RN functions as a Case Manager/Field Staff Supervisor, utilizing their professional skills, which may include and are not limited to: case managing the assigned client’s care, development of the care plan, in compliance with the State and Federal Home Health regulations as well as the Home Health agency’s policies and procedures, and demonstration and teaching of all skilled care (when applicable), personal care and home support activities.

The PD Case Manager is required to visit each client’s home on or before the first day of service to develop and/or review the care plan and to, when applicable, orient/observe the staff providing care. A new client is one who is new to the provider agency, and/or the staff. The RN Case Manager is required to supervise the at least once every 60 days and professional staff at least once every 14 days.

ESSENTIAL FUNCTIONS:

  • Conducts initial nursing assessment through use of the assessment tool and including physical and psycho-social aspects, of patient/family/caregiver in a thorough, accurate, compassionate and timely fashion.
  • Ensuring the staff working in the clients’ home is compliant with AHC policies/procedures.
  • Ensuring the staff in the clients’ home is satisfactorily accomplishing the tasks outlined in the Care Plan.
  • Assessing changes in the clients’ needs and/or circumstances, and reporting these changes promptly to the provider agency, Case Manager or Physician when appropriate.
  • Utilizes own special body of knowledge to provide leadership and guidance to the home health team in formulating and individualizing the plan of care relative to the problems identified, interventions required and goals that must be addressed relative to the patient needs.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent interpersonal skills and communication skills.
  • Excellent computer skills.
  • Willing to cooperate with the staff and respect the policies and procedures of other departments
  • Access to reliable transportation

 

 

Job Requirements

 

Education:

  • Graduate of an accredited nursing program, Baccalaureate degree in nursing or related field, preferred

Experience:

  • Minimum of 2 years clinical nursing experience in general nursing, with Home Health experience preferred.
  • Demonstrates excellent written and verbal communication skills.

Licensure, Registration, and/or Certification:

  • Current license as a Registered Nurse in the Commonwealth of Pennsylvania
  • Cardio Pulmonary Resuscitation (CPR) certified required
  • Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with PA laws.
