Job Description

The Transitional Care Unit (TCU) of the Madlyn and Leonard Abramson Center for Jewish Life provides short-term nursing, medical, and rehabilitative services to help patients continue their recovery process post hospitalization.

The TCU currently has an exciting opportunity for a Full-Time RN Care Coordinator on the 12-hour 7:00pm-7:30am shift.  FT positions require every other weekend (both Saturday and Sunday) and half of all scheduled holidays.

This position is responsible for the management of the floors including assessment, planning, directing, evaluating and coordinating activities in order to provide the highest level of care for those patients.  

Additionally, this nurse will:

  1. Provide direct patient care, direction and supervision to other members of the patient care staff in accordance with the established treatment plan.
  2. Routinely exercise independent discretion and judgment in overseeing the clinical care of patients and administrative duties.
  3. Communicate pertinent information regarding patient status with physician, TCU team and other departments as necessary
  4. Review and update treatment and care plans on a regular basis
  5. Consistently keep TCU Clinical Manager informed of changing patient or unit conditions

Job Requirements

Minimum Experience: We are looking for PA licensed RNs with strong acute or subacute clinical skills; a minimum of 1 year of experience in post acute, acute rehab, or subacute care and who wants to work in a fast paced dynamic department.

Excellent clinical and assessment skills required to care for our patients with complex medical problems. CPR certification preferred but will provide for otherwise qualified applicants.

Minimum Education: Graduation from a Commonwealth-approved School of Nursing.  Valid PA RN License Required.
