RN CARE COORDINATOR - Wynnewood, PA

Exciting Full-Time RN Care Coordinator opportunities available at the Abramson Center’s Transitional Care Center (TCC) located on the campus of Lankenau Hospital in beautiful Wynnewood, PA.

  • The TCC is looking for dynamic, service-oriented Full-Time Nurses for the 8-hour 3pm-11:30pm shift.  The Care Coordinator routinely exercises independent discretion and judgment in overseeing and providing the clinical care of patient duties.
  • This position is responsible for care management and delivery including assessment, planning, directing, evaluating, providing and coordinating activities in order to provide the highest level of care for short-term nursing, medical, and rehabilitative patients. This role will also supervise departmental CNAs.
  • The ideal candidate will have acute, sub-acute or rehab care experience as well as effective verbal and written communication skills and excellent clinical judgment and decision making skills. Our nurses work with low staff-to-resident ratios, have the opportunity to lead and supervise staff, and work in an energetic and fast paced environment.

The Abramson Transitional Care Center is an exciting new launch located on the Lankenau Hospital campus in Wynnewood, PA.  Our expert Transitional Care team has built a solid reputation for passion, quality and excellence. The TCC is currently licensed for 22 sub-acute beds with plans to expand the number of beds this year!

Job Requirements

EXPERIENCE, EDUCATION, AND LICENSURE

Minimum Experience: PA licensed RNs with strong acute or subacute clinical skills; a minimum of 1 year of experience in post-acute, acute rehab, or subacute care and who wants to work in a fast paced dynamic department. Excellent clinical and assessment skills required to care for our patients with complex medical problems. CPR certification preferred but will provide for otherwise qualified applicants.

Minimum Education: Graduation from a Commonwealth-approved School of Nursing.  BSN preferred.

Licensure/Certification: Current Pennsylvania RN Licensure.
