Job Description

Abramson Homecare currently has a need for a full-time RN Case Manager. This position will split time (50/50) between Medicare-certified Home Health cases and Private Duty cases.

This high-profile role provides and oversees patient care for in-home services. As a Home Care RN you would be responsible for: assessing the health status of assigned residents; implementing and evaluating individualized nursing interventions; and coordinating care delivery. Rotation of after-hours/weekends on-call required.

Service areas include, but are not limited to: Lansdale, North Wales, Elkins Park, Glenside, Huntington Valley, Northeast Philadelphia, and “the Mainline.”

The RN Case Manager will rotate with the other nurses between taking night and weekend on-call duties for both Private Duty and Home Health divisions of Abramson homecare.

Job Requirements

The ideal candidate is organized, versatile, and driven. She/he has excellent attention-to-detail, time management skills, and is comfortable working autonomously.  In addition, someone well-versed in the Oasis documentation process would make a great candidate.  Strong computer, communication (written and verbal), and nursing documentation skills are a must.

Minimum Experience Required: 2 years skilled home health experience as an RN.  Hospital experience a plus. Geriatric experience preferred.

Minimum Education: Graduation from a Commonwealth-approved School of Nursing.

Transportation: This position involves working in the community so a valid PA driver’s license and reliable car is required.

Licensure/Certification: Current Pennsylvania RN Licensure.
