Abramson Hospice currently has an exciting opportunity for a Full-Time Hospice Intake Coordinator.  The Hospice Intake/Team Coordinator supports the hospice operation through efficient processing of referrals and admissions.  Responsible for maintaining Medical Records, answering the telephone, assisting with the coordination of Team Meeting, acts as liaison between patients, families and staff. 



Responsibilities of the Scheduling/Intake Assistant include:

  • Maintaining Medical Records
  • Discusses Hospice with interested parties via telephone and arranges for the signing of hospice election forms. 
  • Schedules meetings with patients and families as needed to introduce hospice.
  • Mail packets of information to family members as needed.
  • Maintains regular follow up with referred patients and their families in collaboration with Administrator and Director of Clinical Services.
  • Other duties as assigned. 
  • Set up files for new patients and enter into computer as appropriate.
  • Assemble blank charts for staff use.
  • Working knowledge of computer systems.
  • File accurately using chart order or file order.

Job Requirements

Minimum Experience: Health care experience utilizing office equipment, including computers, for word processing and data entry. Experience with e-mail and electronic spreadsheets. 

Preferred Experience: One year of Hospice intake, medical records maintenance and staff scheduling experience. 

Minimum Education: High School Diploma or Equivalent.

Preferred Education: Some college

Licensure/Certification/Registration: Must be able to travel for work-related assignments and training.
