Job Description

An exciting opportunity for a Nurse Practitioner is currently available for expanding services within our Palliative Care program.

The Nurse Practitioner plays a pivotal role in the coordination and delivery of community-based palliative care to patients with serious life-limiting illness and their families.

Additionally, this position provides comprehensive care through consultations, assessments, care coordination, advance care planning, and family meetings.

This position is a great opportunity to work 1:1 with a supervising Medical Director for the Palliative care program.

The primary shift for this position would be 8:30am -5:00pm, and evening on-call hours may be required.

Job Requirements

Minimum Experience: 3-5+ years of experience as a CRNP with experience in geriatric, palliative, hospice or other advanced complex illnesses. Graduate of a CRNP program required.

A valid driver's license and reliable car is required for this position.

Licensure/Certification: State certification, license and prescriptive authority. Within one year of hire must obtain certification in palliative care
